What should I buy first if I'm starting from scratch?

Scales and a thermometer. With those you are already formulating for real; the rest are conveniences you can add later.

And the spatula, is it really that necessary?

More than you would think. Scraping the bowl properly can be 5 % of the batch, and that 5 % is money and it is ice cream.

Is a fine strainer worth it?

Yes, and it is one of the cheapest things you can buy. Strain the mix before churning and it takes out the milk powder lumps you would otherwise feel in your mouth.

What breaks most often?

Bad whisks and cheap silicone spatulas, which get cut on the rim of the bowl. That is where spending a bit more pays off.
